| Root Cause(s) | Observed one of the pour crew filling a Confilm spray tank from the faucet at the northwest corner of Bed 96. The faucet nozzle is about 12 inches higher than the tank fill cap. Water was spraying onto the floor, which is right at the man door to the bay, posing a slip hazard. |
