| Root Cause(s) | Observed Daniel Sanders walk from the Plant Maintenance Shop to the Batch Plant without PPE. I called out to him to have him put on the appropriate PPE. He acknowledged me, then jumped in the bobcat without putting any PPE on. He did not have a vest, safety glasses, or hardhat. |
