| Root Cause(s) | Employee / Ryan Smallen was carrying tools on the 10th floor of the Bower residents when he hit an ice patch and slipped. The slip caused him to land on his left leg awkwardly. With this being said, Ryan felt tightness and soreness in his upper left ankle and calf. Slight swelling. |
