| Root Cause(s) | A double tee form was removed that exposed an open steam trench that was located under the bed. The open trench had a small cones in each corner which is inadequate protection around an open trench. We will be producing a slab to cover the opening but in the meantime Maintenance was instructed to place barriers around the open trench. |
