| Root Cause(s) | Employee on the stadia form accidently sprayed himself in the face with carb cleaner while cleaning some of the steel rails. |

| Corrective Action | Employee was wearing safety glasses which helped protect his eyes, however we had him use the eye wash basin to rinse his eyes/face multiple times since he said the eyes felt a little irritated. I checked in with him hours later and he said he was fine with no irritation to the eyes. |
