| Root Cause(s) | While Walking through the storage area I discovered 4 welded rebar embeds, with the rebar sticking straight up, that could be sever to fatal if fallen upon or into. I turned the 4 rebar on its side to mitigate the issue and wanted to make sure that this is addressed in the next safety meeting. |
