Root Cause(s) | 4 foot machine had a Hydraulic hose leak Friday. The plane was to replace both lines - Been 4 to 5 years since the last change out - We made the hoses internal in the Maintenance Dept with the newer Hose clamp equipment - The Maintenance Tech followed the information on the crimp machine on which tools and settings based off the data sheet. Location of the fitting was not defined, and no one measured the crimp OD to ensure the crimp met expectations. We ended up with 2 leaks, the first one Sprayed the Tech in the face - Safety Glasses on! -Cleaned him up and the Maintenance Manager pulled the Maintenance Members together to review the process - Found many of the Techs did not know the proper process |
