| Root Cause(s) | A beam is scratched. It looks to be by a Travelift. To my knowledge no one reported the incident. The scratches match a machine where the operator was just above scratches that match the beam. This is an operator issue because the groundperson should be on the opposite side |

| Corrective Action | Have every supervisor of TL operators and other machines speak to said operators and ground men. The message should be that stop and report issues if something happens. Watch where you a driving. Ground men watch the other side. Hard to determine who might have done this. It was not done by the day crew with that TL because he has not been in that area with that machine recently |
