| Root Cause(s) | Tracy Roberts was moving sides on the B5. After setting down and bolting in the last side on the west side of the form, he unhooked the south crane and tried to raise it up and move it out of the way. He had the wrong remote in his hand and was raising the north crane which was still hooked up to the form. The crane let loose and broke the safety brake on the cable. |
