| Root Cause(s) | Earlier in the day our main air compressor went down with a blown piston. Mike Tomczak was in charge of swapping out the compressor. Upon first inspection, without being reminded, Mike had used his lockout lock and all lockout procedures were being properly followed. Great job as this is a 480V piece of equipment. |
