| Root Cause(s) | Settling tank cover in south runway, west end of Plant 1 is bent and flexes when walked on that could cause a trip hazard. Overall the cover is not flush to the floor which is should be closer to prevent a possible trip hazard. |

| Corrective Action | The cover needs to reviewed to see if it is only a bent cover or if the concrete it sits on is deteriorating. The cover needs to be designed heavy enough to withstand the truck and forklift traffic that travel over it frequently. |
