Root Cause(s) | Employee was bringing in a pallet of rebar in the west center door to place next to the bed for the next days production. While bringing the long bars in they had to lift the load high enough to clear the stressing guards on the west end of beds 2 and 3. Because there were 2 pump sprayers left in the center of the drive isle the forklift operator dismounted the forklift to move the sprayers so they could drive down the isle. When they dismounted the forklift they left the load about 9 feet high and partially walked under it to move the sprayers |

Corrective Action | Had a conversation with the employee and told them not to dismount the forklift with the load elevated. There were employees in the area that could have easily been asked to move the sprayers so the operator didn't have to dismount with the load elevated. Additionally, the sprayers should have been noticed before the load had to be elevated so they wouldn't have had to stop while they had the load in the air. |
