Root Cause(s)

When testing haunches for R&D purposes, one end of the beam slipped off the support causing it to slip. We always made sure that the area was clear when the haunches were being tested. No one was hurt and no property/equipment was damaged.

Corrective Action

We stopped testing after this happened and went back to the drawing board. We came back and implemented a different brace for the beam so that it would not be able to slip off. See corrective action image
