Root Cause(s)

A safety pin fell onto the bed of the truck from the area of the crane hook as the driver was climbing down from the front of his trailer after sweeping off garbage. The pin and the keeper was reinstalled.

Corrective Action

As described by the erection contractor, the root cause was due to the pin keeper not being fully engaged. The location of the keeper is inside the auxiliary block and its location makes it very difficult to fully engage. Moving forward, when the pin is installed at ground level and the block is raised, the installer will inspect at eye level to confirm the keeper is engaged. A toolbox talk will be created to communicate this situation to the crew.
