| Root Cause(s) | Cable laying along the B5 that wasn't long enough for B4 was being driven over by the B4 crew with the fork lift, while they were loading a new cable reel in the rack. |

| Corrective Action | Although the cable could've been used on another form, they considered it junk and were driving over it. I told then that we could've used that cable on a different form, but beings they drove over it already to make sure they get it cut into chunks so no one used it. |
