Root Cause(s) | While tensioning the top layer of strand the hydraulic ram was not level. This created a binding issue inside the tensioning chamber on cylinder assembly. The chuck folded back the safety guard on top of ram and safety switch was damaged. Machine was immediately removed from service. |

Corrective Action | Crew operating the tensioner was questioned and schooled on what the root cause of the failure was. Maintenance was called and machine was put out of order for immediate repairs. The final safety switch hookup is in progress and testing will be done the morning of 1-3-18 |
