When we were lifting piece WAI-5104 from job 7220 and the drawing says it should weigh 56,450 and the weight on the scale was over 63,000 after you subtract the rigging the piece weighed 4,000lbs + more then it should of. With the unexpected extra weight we were over chart and once the piece was in the air Travis had to boom the crane up suddenly because he felt the riggers starting to come off the ground luckily he was able to do it without damaging the crane or the piece. This is a problem we keep having with insulated wall panels, I have brought it up to multiple engineers and drafters in the past and I feel like it is falling on deaf ears. I have been told in the past that they think our scales are off but the guys have called out to the field and they are getting the same weights on their scales as we are. We have also picked up the cranes counterweights in the past and are within a few hundred pounds. we are not having this problem with any other piece types just insulated panels

Did not re-ratchet 3 sheave block to trailer
Driver pulled out and block rolled of trailer to the ground
No one was near the trailer when the incident happened

Great communication between forklift operator to place braces in bundling rack and band all 16 together.
Great team work.
Watched them communicate with truck driver of where he want bundles placed on the trailer.
