Misael (yard/load) was carrying dunnage blocks and walking across a hollow core plank used for base when he sprained his ankle. He was told to put ice on it, and elevate it when he got home. It is still sore today, and can't get a boot over the ankle. It is swollen but not discolored.

"Arm" on bed that holds the ram is too short. The guys on the bed are having to tension cable at an angle while using the "arm". This caused the cable to slip in between two of the jaws, not allowing it to work properly. Luckily nothing happened and it held. This happened on 10/23/17 and was noticed on 10/24/17

It was about 6:20 am on Friday morning (10/6) and I noticed loading crew unloading some beams and stockpiling them west of the bridge building. The only lights that were near them were the ones on the front of the yard truck. Those lights were on the trailer with the beams only. There were no lights on the guys that were standing in the stock pile waiting for the beam. This was done by the crew that was operating the yellow yard crane.

Damage to DT while yarding.
Ground guide did not properly clear the landing sight for a DT. Spare piece of 4" x4" x4' wood dunnage was left resting in a vertical position. Crane operator, after being given the all clear by the ground crew, attempted to land the DT. In this process the wood dunnage pushed the DTs deck up and created 3 large breaks. DT was left in place for QC and Safety review. Ground crew will be counceled on incident and additional training will be given on proper landing proceedures for DT product for all Rosemount ground crews.
