Beams were stripped out/loaded onto trailers without having a strap going across them. The height of the beam was exactly 2X the width. Even though this was close to our rule of thumb of not needed a strap, there still should've been one on the beam. This was addressed to the necessary people immediately after the issue was found.

The 1/2" diameter was getting pulled from the reel and it started to unravel itself within the reel. Luckily it was put in properly and no one was hurt. However, if this had not been the case, the strand could've continued to unravel itself and fly around unpredictably.
