When on busy jobsites such as Project Force, Make sure you understand your surroundings when using motorized equipment such as boom lifts. There are trucks, atvs, backhoes digging everywhere you look and could be a hazard that changes minute by minute.

During stressing an employee walked by bed being stressed, stressing operator saw this employee and tried to get his attention but was unable to do so. Operator stopped until employee passed.

He cautious when using boom or other heavy equipment.

While cutting mesh, a new employee received scratch on arm. We will order cut resistant sleeves to avoid another injury.

There is a hole in the compressed air hose next to FB5. It has been leaking compressed air the whole morning and maybe longer. I turned the main valve off. We will need a new hose or have it patched before pouring on FB5

when backing in trailer on wet cast side hit retaining wall with the front bumper. he was trying not hit the rebar that was set out for todays pour.

While driving through yard i witnessed an employee standing on new double bar lift by cab while operator was handling product. I stopped and informed said emplyee that it is a distraction for operator and unsafe for them to be up there to begin with

In QC Lab Cylinder Breaker Just has a red tag saying do not use.

Gradall was park with forks tilted up in the air not flush to ground. I fixed issue.
