Pour 604 of job 7361-Hudson Medical Center included a strand pattern that was more than what the horizontal weldments on the FS39 had the capacity for (total stress on form was 403K). The spandrel jacking plate was needed to accommodate this design. This was well communicated between engineering and production as it was brought up more than a month prior to the pour to be sure we could safely achieve this. There was good communication/cooperation by everyone involved on the production side (plant engineer, production supervisor, quality control, metal shop, team lead, and laborers). Because of the proper planning and execution, we were able to safely produce this design. Great work all around by all parties involved.

When I noticed the task was being performed.

Salvador G. found a nylon strap that was damaged. He immediately contacted the appropriate personnel to address the situation. The strap was then red tagged, taken out of service, and thrown away.

Area 3 Grids PB & PC between grids 1 & 2 Stair #1 air vent in all level 9' x 3'
This hole in the (2) DDT 's deck was coved with 2"x 4' x 8'/ 12" on center with 3/4" 4' x 8' plywood marked with Red painted HOLE on top.
Grouter went thru and did this task and also pulled the ladders for our second point of egress on all floors and covered up those hole as well with plywood also marked HOLE in Red Paint
