A stress head was removed from a self-stressing form that is no longer in use. Once the stress head was removed the anchor bolts were exposed that created a trip hazard.

Witnessed employee jumping from bed 4 to the floor on waste strand. Not best practice, should have used steps.

Witnessed an employee not fully using a face shield correctly while cutting scrap strand.

Employee walking down runway in plant 1 safety glasses in hand until he seen me and rapidly put them on.

New employee crossing bed 1 while stressing the bed. Employee was reminded of the hazard, as well as the training he had received.

One employee was loading a trailer with columns from the south flat form. While the employee was moving the load and guiding the load with a stick to place the load properly on the trailer, another employee was moving scrap strand by dragging the scrap behind him he walked behind the crane operator with strand in tow. The crane operator stepped on the strand causing him to nearly lose his balance.

Witnessed stripping crew truck back over a pallet of PVC tubing breaking three of the tubes and crushing part of the pallet.

The Walls Crew noticed that the Rigging Pickers used to stockpile/ship walls, were getting worn down and could possibly lead to a failure. The pickers have been taken out of service.

Almost hit overhead door in east bay of building b with forklift.

Plastic handle broke off on safety gate for strand on tensioning ram.
