Another trade took one of our ladders down in order to accomplish their task. This ladder was Wells only access point to level 1. If there was an emergency causing us to vacate level 1 in a hurry, our missing ladder wouldn't of allowed us to do so.

When following up on our recent rebar amputation, I checked out the Rebar Bender area. I was looking for a posting / guidance that instructs our four employees on the corrective action taken. Specifically, I was looking for the hook that would be used to grab the stirrups and like products rather than using the hands. I was unable to locate the hook and find any reference to this process. I stopped Jacob and showed me where the hook was stored - behind a workbench with only the end of the hook being visible.

I observed two 16 AWG gauge home style extension cords connected together to power a light of other equipment. This type of extension cord does not have the capability of grounding and not suggested for use in an industrial situation.
