Since this device was installed in the yard, we are unaware of the embedment depth, amount of epoxy, and whether or not the holes were cleaned out well enough prior to applying the epoxy. They were unloading deck plank on steep hill leading into basement. The crane operator picked 2 pieces of plank and started to raise the load off of the trailer. Because of the incline, the end of the load closest to the crane lifts off the trailer before the front end. The stack was dragged for a bit until it is completely off the trailer. As the load clears the trailer, the truck starts to slide down the incline. The driver hollered for the crane operator to put the weight down onto the trailer. he did but it continued to slide until the trailer made contact with the crane hitting the step. There were no injuries to personnel. When the load was lifted off the trailer, it pulled the truck suspension up taking most all of the weight from the truck allowing it to slide. | Needs Attention | View / Edit Details | ||
