Jobsite access for trucks extremely poor, potential for a lot of jobsite damage and trucks getting stuck. The access route is difficult for trucks(sharp turns and too close together) with too many obstacles. Rain day prior left site in extremely poor condition. When the sweeping machine is dumped a dust cloud is created in the that area. One option is to dump direct into the compactor. Another option is to put a vacuum on the dumpster and capture the dust during dumping.

Gregg Jacobson 32" DT form from GF05/10/20189Safe Equipment Operations

We relocated the 32" form from GF to Rosemount. The first project called for depressed strand and the MG folks never depressed at Hanson. We had Tom Holmes go to Rosemount to demonstrate our process. The depression frame and Jack was shipped to the Rosemount plant along with the from. The depression shoes where ordered for the Wells 32" form and jack system. The shoes did not fit properly on the jack. They depressed the strand and it was noted that the strand did not stack properly in the shoe. Tom contacted me and we immediately shut it down and had the proper shoes made up... They are presently pouring with the new depression shoes that fit the ram from GF.

The use of Tapcon fasteners. If you still the 3/16 hole required for 1/4 inch tapcon. It still takes a lot of force to put them in even if you clean the hole out a couple of times. Witch may be weakening the Tapcon or partially breaking them.
