The ground man left a strap connected to the trailer on top, but disconnected the other end of it. The operator then picked the piece with the strap still strung over the piece. There is a risk of the strap getting hung up with the rigging on top of the piece and causing an issue.

One of my operators called me over to look at a stack of rib slabs that got put away last night, the dunnage on the rib slab at the bottom of the stack did not line up with the other 3 stacked on top of it creating a big crack through the deck.

Brett called me out to look at a DT in row-3, the deck was broken due to incorrect dunnage placement, this piece was set here within the last couple of nights. when they stacked the T on top they did not line up the T blocks with the stems and the weight of the T caused the bottom piece to crack.

It was observed (today) that during the thaw yesterday dunnage had sunk into the mud causing one wall panel to sink forward and down. This lead to the top of the wall to shift out of the bracing by about 3/4".

While Patching/Detailing TT's wood with nails sticking out/up were noticed on the ground in a pile around the area in which the TT's were being worked on. I spoke with the patchers and let them know they need to get them up off the ground in the work area so no one steps on a nail on accident.

After a trailer was loaded with a TT, the driver was heading to put trailer in the Shipping Line. When stopped and about to start backing up the driver noticed a wobble and then the TT started sliding off the trailer and the to the ground.
