Ryan Paulouski | Midwest | Field | 419220 Empirical Foods | 12/02/2021 | 0 | Safe Operation | While picking a piece with a John Deere part, the device started to pull out/off the piece as we were in the rotating process. Since this device was installed in the yard, we are unaware of the embedment depth, amount of epoxy, and whether or not the holes were cleaned out well enough prior to applying the epoxy. None the less, this piece (WSS-3250) only weighted 23k we were able to get the piece vertical. May want to look into this a little more. | Needs Attention | View / Edit Details

Kevin Lesto | Midwest | Field | 7346 | 05/11/2022 | 9 | Unsafe Act | Is there any reason why these connections weren't on the top of the flat.. Luckily ew were able to find a lift.. 30' in the air off a ladder isn't only not any fun "it's not very safe". Being in a Creamery you have the problem 9f welding around Stainless Stell Equipment which adds a least twice the welding time because we normally have to take the time to cover the Stainless, and normally take 2 worker instead of 1. When the plate connection is on top its much more convenient for everyone involved and much safer | Needs Attention | View / Edit Details

Geoff Norrena | Mountain States | West Division Plant | All bed locations (inside and outside) | 07/26/2021 | 7 | General Communication | We use computers to record and transmit data to the QC Lab, these computers are in yellow plastic boxes. I collect them everyday in the morning so we can charge them and reuse them. There are orange hooks at each bed where these boxes are to be placed before the bed is stripped. These boxes are not being put on their hooks. Boxes are being damaged, and often go missing. Please make sure that whichever person cuts the wire is now responsible to put the box on its designated hook. | Needs Attention | View / Edit Details
