New operator was moving the plotter to the assigned parking lot. The unit was swinging more than usual. Operator tried to stop the swinging and almost was thrown off the bed

Man lift has no lights when moving before/after dark. Creates a hazard for the employee driving it and for other vehicles not being able to see it.

While heating up a a hot rod to burn foam the torch was right next to the swept pile of foam on the floor, a spark went off the end of the rod and caught the pile of rubbish on fire. By the time we got it put out the flames were 5 ft tall coming off the pile

Forklift operator was doing his daily inspection and found that the seat belt was connected. I'm assuming that someone has done this so that they don't have to listen to the alarm going off.

A driver was chipping out concrete on the top of his truck, no ear plugs. I offered him some, he refused. It was really loud even 20 feet away.

One of my operators called me over to look at a stack of rib slabs that got put away last night, the dunnage on the rib slab at the bottom of the stack did not line up with the other 3 stacked on top of it creating a big crack through the deck.
