lots of grinding on a panel at bower residence due to panel placement. arms and hands numb and had to take multiple breaks. still affecting me to this day. lots of jobs require grinding-cutting with small hand held grinders. talked to tom peterson on december 1 about ongoing issues and told me to fill out a see report. i have pictures on my phone of the work we were doing and cant get to this. hands and arms go numb at random times and when grinding, cutting and other tasks

I took him to the eyewash station and showed him how to use it, and told him techniques to help. stayed with him until he was recovered. gave him a personal eyewash container (from a first aid box) to keep in case the eye gets bothered again.

Employee was bringing in a pallet of rebar in the west center door to place next to the bed for the next days production. While bringing the long bars in they had to lift the load high enough to clear the stressing guards on the west end of beds 2 and 3. Because there were 2 pump sprayers left in the center of the drive isle the forklift operator dismounted the forklift to move the sprayers so they could drive down the isle. When they dismounted the forklift they left the load about 9 feet high and partially walked under it to move the sprayers
