Lots of small piles of concrete on the ground by every bed that pours.

Loading scrapped concrete into the truck to haul it away. the truck is parked next to the port-a-potty.
a 500pg piece of concrete being loaded into the truck fell out of the front loader scoop and landed 5 feet away from the toilet.
if it hit the toilet and someone was in there, a serious injury would occur.

Flammable aerosal cans were left out on the ground to be run over and left in plate baskets.

Bed Leadman got debris in his eye, came into the QC Lab asking for help. I took him to the eyewash station and showed him how to use it, and told him techniques to help. stayed with him until he was recovered. gave him a personal eyewash container (from a first aid box) to keep in case the eye gets bothered again.

Crew has completed restacking product in row-1.

People who fill up machines with fuel and gas need to pay attention and not spill fuel on ground

Stripping crew leaving nails sticking up without bending them over. This has been a reoccurring issue.

Someone Parked a steam roller directly in front of the fire-hose spigot

Erected a 36'-8" tall load bearing column missing the (2) v-pocket when it was produced in the plant.
Had to re-shore in the morning with a B-5 from the column horizontal to the shear before we removed the shore on the 2nd to erect the 3rd floor DDT"s.
Once we loaded the 3 DDT's on to the beam supported by the column it twisted the column and pulled the column out of plum by 1 1/2" Bending the B-4 shore that was supporting it East and West
The fix we had to replace the (2) missing V-pockets did not hold the tall column in place

Loaded trailer dropped without drop blocks under dollies.

Light and chains need to be on for cut down, stressing, or destressing. In the morning no chains or light were on in the or outside of the casting bay.

Visited jobsite and found it to be in great shape and very clean and orderly.
