Observed one of the pour crew filling a Confilm spray tank from the faucet at the northwest corner of Bed 96. The faucet nozzle is about 12 inches higher than the tank fill cap. Water was spraying onto the floor, which is right at the man door to the bay, posing a slip hazard.

A Sealants Crew was conducting work on a roofline. Joint height was 18', a ladder was being used to complete work. The rubber roof membrane had been installed, river rock had not been placed. Someone went up the ladder while someone stayed at the bottom to hold the feet. The feet of the ladder slid away from the wall. The person on the ladder rode the ladder to the deck. He sustained no serious injuries, just a few scrapes and a bruise on his thigh. The person holding the ladder at the bottom was sent sliding a few feet. He sustained no injuries.

A double tee form was removed that exposed an open steam trench that was located under the bed. The open trench had a small cones in each corner which is inadequate protection around an open trench. We will be producing a slab to cover the opening but in the meantime Maintenance was instructed to place barriers around the open trench.

Santiago notice that the ladder bolts on the East silos were not tightly put on. The ladders are rarely used, if not not used at all to access the top of the silos. He also said that the rope grab that is on one of the vertical lifelines is not functioning correctly because of concrete debris.
