Doug Kler send Huan Anderson and Antonio Brito to install backer rod on 85" boom lift.
In this are there were PVC pipes stubs extended up 8" above the concrete slab to clear the bottom of the boom lift
Boom lift was in a position to reach every area they needed to work
On Saturday I walked the area and found that the lift had was moved and found 4 broken pipes with 1 broken pipe under the tire.
No one reported the broken pipes to Doug Kler or Myself at the end of shift on Friday
Trade Damage is not what we do.
To me this is careless and not paying attention to there surroundings by both employees

4 foot machine had a Hydraulic hose leak Friday. The plane was to replace both lines - Been 4 to 5 years since the last change out - We made the hoses internal in the Maintenance Dept with the newer Hose clamp equipment - The Maintenance Tech followed the information on the crimp machine on which tools and settings based off the data sheet. Location of the fitting was not defined, and no one measured the crimp OD to ensure the crimp met expectations. We ended up with 2 leaks, the first one Sprayed the Tech in the face - Safety Glasses on! -Cleaned him up and the Maintenance Manager pulled the Maintenance Members together to review the process - Found many of the Techs did not know the proper process

A beam is scratched. It looks to be by a Travelift. To my knowledge no one reported the incident. The scratches match a machine where the operator was just above scratches that match the beam. This is an operator issue because the groundperson should be on the opposite side

A double tee form was removed that exposed an open steam trench that was located under the bed. The open trench had a small cones in each corner which is inadequate protection around an open trench. We will be producing a slab to cover the opening but in the meantime Maintenance was instructed to place barriers around the open trench.

A finisher had a water bottle full of epoxy, and left it out in direct sunlight while he went to lunch.
after a long period of time, i happened to see the bubbling bottle leaking all over a weld plate on the bed, thought to immediately get it out of the sun. Turns out the bottle was incredibly hot, and mildly burned my fingers (not badly), even the employee with gloves on said he thought it was super hot too!
dangerous for unlabeled chemicals to be left unattended in the hot sun.
